Azalea Eye Clinic currently accepts the following insurances:

We are currently working to accept additional insurances.  If we are currently not an in-network provider for your insurance, we will still be glad to schedule you for an exam.  In these cases, the patient will pay for services at the time of the exam and be responsible for submitting a reimbursement claim to the insurer.
